CONSENT FORM
The Department of Psychology at The University of Cenrtral Oklahoma supports the practice of protection for human subjects participating in research. All experiments are approved by a review board and meet Federal Guidelines Protecting Human Subjects. You have the right to withdraw at any time without penalty. Information about you will be kept confidential.
For each experiment in which you participate the specific instructions and requirements will be explained to you at the time of the experimental session. At the conclusion of the study you will be provided with an explanation of the purpose of the study and the results.
You are free to refuse to participate in any procedure or to refuse to answer any question at any time without penalty. You are free to withdraw from a research Project at any time without penalty.

PARTICIPATION IN PSYCHOLOGICAL RESEARCH
Participation in psychological research is a course requirement. The reason for this requirement is to give you an insider's view of the science of psychology. There are two forms of participaticn: You can sign up as a subject in five experiments or you can write an acceptable research paper on a topic approved by your instructor. Time of involvement in a given experiment usually will not exceed an hour; when it does you will be given credit for two experiments. A research paper is expected to be approximately 1,000-1,200 words in length (about 4 double-spaced pages). A student who does not complete one of these options will be given a grade of I (incomplete) for the course.
Those electing to participate in five experiments are responsible for making their own appointments. Research participation sign-up sheets are posted on the red bulletin board in the middle of the hall on the fourth floor of Old North. Please read the sign-up sheets carefully to see if any restrictions apply. When you sign up for an experiment note carefully the following information: where the experiment is, when it is, and the name and telephone number of the experimenter If you find you cannot participate in an experiment you signed up for, call the experimenter immediately or at least 24 hours in advance to cancel. If you Simply do not show up for an experiment you are signed up for, or do not call to cancel you will be declared a "no-show" Each "no-show" will result in an additional experiment you must participate in beyond the required five experiments because the time and effort expended by experimenters to accommodate your participation is very extensive.
After participating in an experiment, the experimenter will give you a signed card entitled General Psychology Receipt for Research Participation. This card is proof of your participation and should be kept in case there is an error in accounting on the part of the course's accountant. For experiments scheduled longer than one hour, you should obtain a signed receipt for each hour or fraction of an hour of participation.
If the experimenter fails to show up for an experiment for which you have arrived on time, please notify the departmental secretary Old North 401, Extension 5708 in the Psychology Department office.
